Seniors are at high risk for serious falls. Our balance and fall prevention program is founded on evidenced-based strategies and focuses on reducing the frequency of patient falls and reducing hospital re-admissions for injuries from falls. Our prevention plan is through patient/caregiver education re: home safety, environmental modification, medication management, and evaluation and treatment to improve balance and strength.
VRNS prevents falls with the following interventions:
All patients are screened for the risk of falling, utilizing evidence-based tools
Home assessment for environmental modifications and equipment
Medicine assessment and monitoring
Interventions based on screening findings
General education and home safety instruction
Patient and caregiver education with materials for continual reinforcement
Referral to community resources: Medical alert systems
Interdisciplinary collaboration (MD, nursing, physical therapy, occupational therapy, medical social worker, community resources) for enhanced patient outcomes
